Medical records are held partly in written folders and partly on computer.  All members of our team have access, as needed, for them to do their job effectively.  With your agreement we include information about you in referral letters to Hospitals, etc.  Auditing of our work takes place by the CCG.  Your written consent would be needed before giving any other person information about you or your family.

Summary Care Record

The NHS in England is using an electronic record called the Summary Care Record (SCR) to support patient care. The Summary Care Record is a copy of key information from your GP record. It provides authorised care professionals with faster, secure access to essential information about you when you need care. Summary Care Records improve the safety and quality of your care.
